Phase 2. SCOPED TO THE APP IN THE PATH, not a bare delete by id: the entitlement must belong to {id}, and a well-formed pair that does not match is 404 not_found. So a stale admin page cannot revoke an entitlement on some OTHER app by replaying an id it still holds, and the URL means what it reads as. Revoking an ‘all’ row does not revoke anybody’s personal grant, and vice versa. A granted_by=‘provider’ revoke is NOT permanent - the next library sync re-grants it if the title is still installed; the permanent fleet-wide suppression is the Phase 4 ignore rule. Written to the admin activity log as app.entitlement.revoke.
